Swine Influenza| Symptoms, Diagnosis and Treatment

Swine flu is a respiratory disease caused by the viruses that particularly infect the respiratory tract of the pigs. It results in nasal secretions, dry cough, decreased appetite and the list goes endlessly! The symptoms of swine flu in pigs are very much similar to those caused by human flu in humans. In most cases, humans developed swine flu infections when they were closely related with pigs. The disease is spread among pigs through direct or indirect contact, and also from pigs that are infected but do not show symptoms. To prevent pigs from this contagious diseases, pigs worldwide are vaccinated.

Is swine flu contagious?

The swine influenza virus usually comes from HINI type. The other types include H1N2, H3N1, H3N2. It is important that the virus is a combination of flu genes from birds, human flu & swine.

There is a common misconception that swine flu is developed from eating cooked pork products. This is definitely not! It is spread through either ingestion or inhalation of droplets through sneezing or coughing from people who have already been victims of swine flu. The newest swine influenza that has caused swine flu is the A H2N3v. In this “v” means the variant which means originally it infected only pigs but now it has begun to infect even humans.

The incubation period for swine flu is normally 1 to 4 days with the average being 2 days. In some cases. The incubation period may last for 7 days in both adults and children. The contagious period begins one day ahead before symptoms arise in the individual and usually lasts for 6 to 7 days. In people with weak immune systems, the contagious period may last even longer for 10 to 14 days.

SYMPTOMS OF SWINE FLU:

The symptoms of swine flu are similar to other influenza infections. It includes:

These symptoms last for about a week or even longer if the person suffers severe infection. The methods of managing these symptoms are similar to the regular flu. Give your body a sound sleep and good rest this enables the immune system to fight against the viruses. Hydrate yourself every now and then. Drink plenty of water, soups and other juices that can provide a lot of nutrients to your body.

A vaccination has been invented to fight swine flu. Originally, it was treated as pandemic diseases. Now, it has changed to seasonal flu. DIAGNOSE & TREATMENT:

There are a variety of viruses that cause swine flu. A diagnose may be done by taking a sample from your body. A sample is taken by swabbing your nose or throat. This swab is further treated by various genetic & laboratory techniques. Mostly swine flu is diagnosed by noting a series of symptoms.

Most people with swine flu recover soon even without the aid of medications. In very rare& severe cases, they may have to go in for medications. The 2 main treatments are adamantanes and medications that inhibit the influenza neuraminidase protein. The two antiviral drugs recommended for treating swine flu are oseltamivir & Zanamivir. The viruses can develop resistance to these drugs so these are usually referred to people who are at high complications and are unable to fight the disease on their own. Other people who are healthy and are still suffering from swine flu are capable of fighting the viruses on their own.

RISK FACTORS:

Some people are at increased risk of being affected by swine flu. It includes:

  • Adults above 65 years of age  
  • children below 5 years of age
  • People with chronic disease
  • Pregnant women
  • People with weakened immune system
  • Teens receiving long-term aspirin therapy

PREVENTING SWINE FLU:

One best way to prevent swine flu is to take the yearly vaccination. Apart from this, there is some measure which we can adapt to keep ourselves healthy and free from swine flu.

  • Wash hands frequently with soaps & sanitizer.
  • The virus can survive on the surface of tables, telephones, etc. At these times the virus remains inactive. You may touch these palaces and unknowingly keep the hand in your mouth or nose which paves way for the virus to entire your body and become significantly active.
  • If you are sick it is better to stay at home rather than going to school or office for work
  • Avoid going to places with large gatherings when swine flu is in season.

HOME REMEDIES TO PREVENT SWINE FLU:

There is no single step to cure the disease. Hence, you can take many steps at home to prevent swine flu and also to reduce the symptoms when you get to come in contact with the virus. Here are some steps:

CATARACT SURGERY: TYPES, PROCESS & RISKS

WHAT IS CATARACT??

A cataract refers to the clouding of the eye’s natural lens which lies behind the iris and the pupil. It is the principal cause of blindness in the world and is most common in people of age 40 years and above.

There are 3 types of cataract:

  • SUBCAPSULAR CATARACT: This type of cataract occurs at the back of the eye. It is most common in people with diabetes or those taking high doses of steroid medication.
  • NUCLEAR CATARACT: This type of cataract occurs in the deep central zone of the lens. It occurs due to ageing.
  • CORTICAL CATARACT: This type of cataract begins in the periphery of the lens and slowly spreads towards the centre of the lens. It affects the cortex of the lens, which is the part of the lens that surrounds the central zone.

CATARACT- IS IT BECAUSE OF YOUR FAMILY, LIFESTYLE OR JUST BAD KARMA??

Our eye lens works very much like the camera, focusing light onto the retina for clear vision. It also adjusts the eye’s focus enabling us to view the things both close up and far away. The eye lens is made of water and certain other proteins that let light to pass through it. With ageing, some of the protein may clump together and cloud up over a small area. With time, the clouding up becomes larger and larger making it harder to see. This condition is called cataract.

Nobody is pretty sure why the eye lens changes with age resulting in the cataract. Researchers have eventually found out other risk factors that may lead to cataract. Some of which are:

There’s no concrete proof on what could reduce the risk of cataract. There is no harm in practising a healthy lifestyle that consists of a diet rich in nutrients, vitamins, minerals, antioxidants and omega 3 fatty acids. Regular exercise and sound sleep also add on to your quality of life.

Though it seems to be terrific, it is something we needn’t worry a lot about! The advancements in the field of medicine have made it possible to cure the patients of cataract with help of surgeries.

CATARACT SURGERY:

Cataract surgery is the process of removing the natural lens of the eye (crystalline lens) which is affected by cataract. During the cataract surgery, the patient’s cloudy lens is removed and is replaced with synthetic lens ensuring lens’s transparency. The synthetic lens replaced is otherwise called as INTRAOCULAR LENS (IOL). The cataract surgery is performed by an ophthalmologist.

There is two commonly preferred type of surgery for cataract. They are:

  • PHACOEMULSIFICATION
  • MANUAL SMALL INCISION CATARACT SURGERY (MSICS):cataract, healthy eye

Phacoemulsification is the most commonly preferred method for cataract surgery. It involves the use of ultrasonic energy to emulsify the cataract lens. It comprises of 6 steps:

  • ANAESTHETIC: Using topical anaesthetic eye drops or subtenon injection, the eye is numbed.
  • CORNEAL INCISION: two cuts are made at the margins of the clear cornea that allows insertion of instruments into the eye.
  • CAPSULORHEXIS: With help of a needle or a pair of forceps, a circular hole is made in the capsule in which the lens is placed.
  • PHACOEMULSIFICATION: Ultrasonic energy is used to emulsify the lens into a liquid. The resulting emulsion is then “sucked away”.
  • IRRIGATION & ASPIRATION: The cortex, which is the outer layer of the cataract is sucked away. The fluid removed is continuously replaced with saline solution to prevent a collapse of the structure.
  • LENS INSERTION: A plastic, foldable lens is placed in the capsule which contains the natural lens. Along with this, an antibiotic is also injected into the eye to reduce the risk of infection.

Finally, salt water is injected into the corneal wounds, which cause the area to swell and seal the incision.

MANUAL SMALL INCISION CATARACT SURGERY (MSICS): This technique is evolved from ECCE. The word “small” in the title refers to the wound being smaller than an ECCE, although it is larger than a PHACO wound.

HOW LONG DOES IT TAKE TO PERFORM A CATARACT?

If it is an uncomplicated surgery, then it will not take more than 10 minutes. Since the patient will be given anaesthetic while performing the surgery, it might take about 30 minutes to recover from the sedation.

HOW TO PREPARE YOURSELF FOR THE CATARACT SURGERY?

Before undergoing a cataract surgery you may be asked to do the following:

  • A week before the surgery, the doctor may take certain painless tests using ultrasonic energy to determine the size and shape of your eye.
  • You may be advised to stop certain medications at the time of your surgery as that might increase the risk of bleeding during the procedure.
  • You will be advised to use antibiotic eye drops to reduce the risk of infection.
  • You will be instructed to fast for 12 hours before your surgery.

RISKS AND COMPLICATIONS OF THE SURGERY:

A person suffering from cataract alone may not have many complications, but those who have other eye conditions such as uveitis, high myopia (severe short-sightedness). Less than 10% of the people have greater chances of developing PCO, within two to three years after the cataract surgery.

 POSTERIOR CAPSULE OPACIFICATION (PCO): it is the condition in which the lens capsule thickens thereby causing cloudy vision. This isn’t the cataract developing again, it is the skin or membrane growing over the artificial lens

Other complications are less common but to list a few, they are:

During the operation:

  • inability to remove all of the cataracts
  • tearing of the lens capsule
  • bleeding inside the eye
  • a bit of the cataract dropping into the back of the eye
  • damage to other parts of the eye, such as the transparent outer layer of the eye (cornea)

After the operation:

  • swelling or inflammation of the eye
  • swelling and redness (inflammation) in the eye
  • swelling of the retina- where the fluid builds up between the layers of the retina at the back of the eye. This may sometimes affect the vision.
  • Swelling of the cornea- where the fluid builds up in the corner at the front of the eye; it usually clears itself.
  • retinal detachment – a rare complication where the retina (the layer of nerve cells inside the back of the eye) becomes separated from the inner wall of the eye
  • infection in the eye – such as endophthalmitis (a rare bacterial infection)

It is important to seek medical advice if you experience any increasing pain in the eye or loss of vision or redness after the surgery. It is possible to treat these complications with further surgery.

There is also a small risk of around 1 in 1000 may experience permanent damage to the eye resulting in loss of vision. However, the majority of the people lead a happy life with improved vision after the surgery.

  • The production of pregnenolone is initiated in the Leydig cells of the luteinizing hormone.
  • The synthesis of oestrogen in females helps in controlling the growth and development of sexual characteristics in females.
  • The synthesis of glandular oestrogen takes place in the ovarian cells and the corpus luteum.
  • The luteinizing hormone (LH) stimulates the granulosa cells to release pregnenolone.
  • Pregnenolone then diffuses from these granulosa cells and mediate the process of pregnenolone hormone conversion to androstenedione through DHEA.
  • The process of extra-glandular synthesis facilitates the aromatization of androgens to oestrone.
  • The fat cells convert the androgen to oestrone and thereby increases the serum oestrogens.
  • The bones convert testosterone to form local oestrogen and thereby supports the development of epiphyses.
  • Even though oestrogens are usually responsible for balancing the sexual characteristics in females, however, they are also synthesized at some levels in males.
  • Progestin synthesis also exists. The inventories of hormone synthesis alter menstrual cycles during its production.

Sex Hormones: Regulation

  • The release of GnRH regulates the secretion of few hormones like the luteinizing hormone (LH) and the FSH during the movement and circulation.
  • The luteinizing hormone (LH) regulates the release of testosterone from the related cells and stimulates its production.
  • The testosterone released has a negative effect on the luteinizing hormone (LH) and the hypothalamic GnRH
  • The FSH regulates the stimulation and production of inhibin from the related cells.
  • The released inhibin then again feeds an anterior pituitary in return to produce FSH.
  • The regulation of hormones in females may be considered a bit complicated as compared to male hormones regulation. The reason is mainly that of the effects of various hormones that vary during the stages of menstrual cycle.
  • In males, the hypothalamic gonadotropin-releasing hormone (GnRH) leads to the production of LH and FSH next to the anterior pituitary.
  • Feedbacks from the theca cells are regulated by the production of the progestins.
  • Feedbacks from the granulosa cells are regulated by the production of the inhibin.
  • Feedbacks from the corpus luteum are regulated by the production of the progesterone and oestradiol during the menstrual cycle phase.
  • Any higher amount of the production or release of the progesterone and oestrogen takes place due to the corpus luteum. As a result, the release of LH and FSH are inhibited.
  • Further, the levels of few hormones like specifically the LH and the FSH tend to increase to some higher levels and thus a decrease in the levels of oestrogen and the progesterone takes place whereby causing the occurrence of the certain new follicle.
  • The follicle that is in the growing stage starts to release oestrogen in heavy amounts which thereby also affects the release of LH and FSH
  • The sudden increase of LH and the FSH takes place which then prepares for the process of ovulation. This mainly happens due to a certain increase in the production of oestradiol or the new follicle that gets produced to higher levels.
