Nurse took a needle and poured water out of sink into needle, removed IV, and injected the water, is that bad?

Plain water is unsuitable to be directly injected into the vein. A trained nurse would never do that. You must have got some confusion regarding that. She must have used the saline water to clean and flush the canula clot.
