Most often when a woman is not able to conceive or is not getting pregnant, then it is predicted that the women are infertile. However, men play an equally important role in the process of fertilization. Around 1 in every 5 infertile couple, the reason is the fertility issue of men. Low sperm count in men can be one of the major reasons for this infertility.  Around 90% of the times, the reason for male infertility is the problem of low sperm count. As a couple, struggling to conceive for one’s own child is really a frustrating experience and a concern.

  • The condition when your sperm counts are low, i.e., less than 15 million sperm/ml are medically termed as oligospermia.
  • In case sperms are found totally missing and your semen sample is tested to have no sperms at all, then that condition is medically termed as azoospermia.

  • A total sperm count of 20 million or more is a healthy count.

Stomach Ulcers: Before knowing about the symptoms, treatment and prevention of stomach ulcers, let’s first get to know it well!

Stomach ulcers, also known as peptic ulcers, are the painful sores formed in the lining of the stomach or small intestine. A thick layer of mucus protects the stomach from the digestive juices and acid but when this layer is reduced, stomach ulcer finds its way. The digestive acids are thus enabled to eat the tissues of the lining away. A peptic ulcer of the stomach is called a gastric ulcer; of the duodenum, a duodenal ulcer; and of the oesophagus, an oesophageal ulcer. It differs from erosion because it extends deeper into the lining of the oesophagus, stomach, or duodenum and incites more of an inflammatory reaction from the tissues that are involved.

Some of the major causing agents are the long-term usage of non-steroidal, anti-inflammatory medicines or NSAID pain relievers like ibuprofen, etc. Other causes include an infection in the digestive system caused by the bacteria H. Pylori; poor diet; weak immune system; high amount of stress; poor lifestyle habits. Excessive smoking and alcohol intake is also another cause. Ulcers also seem to run in families. Older adults are also more on the risk of getting an ulcer.

STOMACH ULCERS SYMPTOMS:

An ulcer may or may not have symptoms. And the most common symptom is stomach ache or a burning sensation up from the neck down to the belly button. The severity of the ulcer changes the symptoms and the severity of the symptoms; therefore, depends upon the severity of the ulcer. Other symptoms include:

The treatment varies depending on the cause of the ulcer. Most ulcers can be treated with a prescription or a course of antibiotics, or a medication called a proton pump inhibitor (PPI) but in rare cases, surgery may be required. Antacids are used as an effective treatment to treat ulcers as they neutralize existing acid in the stomach. Some ulcers, however, heal even without medications although they can recur as well. Therefore, the major problems resulting from ulcers are related to ulcer complications. Complications include bleeding, perforation, and obstruction of emptying of the stomach.

STOMACH ULCERS PREVENTION:

Ulcers can also be prevented with certain lifestyle changes, few of which are listed below:

  • Don’t smoke.
  • Avoid alcohol.
  • Don’t overuse aspirin and/or NSAIDs.
  • On indication of symptoms of an ulcer, consult a doctor before it worsens.
  • Clean eating habits
  • Maintaining hygiene

Most ulcers caused by H. pylori are completely treatable. But untreated stomach ulcers can lead to more serious problems, like internal bleeding and stomach cancer. If you suspect you have an ulcer, don’t ignore it. Speak to your doctor, make a plan, and get treatment.

  • NUCLEAR CATARACT: This type of cataract occurs in the deep central zone of the lens. It occurs due to ageing.
  • CORTICAL CATARACT: This type of cataract begins in the periphery of the lens and slowly spreads towards the centre of the lens. It affects the cortex of the lens, which is the part of the lens that surrounds the central zone.

Our eye lens works very much like the camera, focusing light onto the retina for clear vision. It also adjusts the eye’s focus enabling us to view the things both close up and far away. The eye lens is made of water and certain other proteins that let light to pass through it. With ageing, some of the protein may clump together and cloud up over a small area. With time, the clouding up becomes larger and larger making it harder to see. This condition is called cataract.

Nobody is pretty sure why the eye lens changes with age resulting in the cataract. Researchers have eventually found out other risk factors that may lead to cataract. Some of which are:

CATARACT SURGERY:

Cataract surgery is the process of removing the natural lens of the eye (crystalline lens) which is affected by cataract. During the cataract surgery, the patient’s cloudy lens is removed and is replaced with synthetic lens ensuring lens’s transparency. The synthetic lens replaced is otherwise called as INTRAOCULAR LENS (IOL). The cataract surgery is performed by an ophthalmologist.

There is two commonly preferred type of surgery for cataract. They are:

  • PHACOEMULSIFICATION
  • MANUAL SMALL INCISION CATARACT SURGERY (MSICS):cataract, healthy eye

Phacoemulsification is the most commonly preferred method for cataract surgery. It involves the use of ultrasonic energy to emulsify the cataract lens. It comprises of 6 steps:

  • ANAESTHETIC: Using topical anaesthetic eye drops or subtenon injection, the eye is numbed.
  • CORNEAL INCISION: two cuts are made at the margins of the clear cornea that allows insertion of instruments into the eye.
  • CAPSULORHEXIS: With help of a needle or a pair of forceps, a circular hole is made in the capsule in which the lens is placed.
  • PHACOEMULSIFICATION: Ultrasonic energy is used to emulsify the lens into a liquid. The resulting emulsion is then “sucked away”.
  • IRRIGATION & ASPIRATION: The cortex, which is the outer layer of the cataract is sucked away. The fluid removed is continuously replaced with saline solution to prevent a collapse of the structure.
  • LENS INSERTION: A plastic, foldable lens is placed in the capsule which contains the natural lens. Along with this, an antibiotic is also injected into the eye to reduce the risk of infection.

Finally, salt water is injected into the corneal wounds, which cause the area to swell and seal the incision.

MANUAL SMALL INCISION CATARACT SURGERY (MSICS): This technique is evolved from ECCE. The word “small” in the title refers to the wound being smaller than an ECCE, although it is larger than a PHACO wound.

HOW LONG DOES IT TAKE TO PERFORM A CATARACT?

If it is an uncomplicated surgery, then it will not take more than 10 minutes. Since the patient will be given anaesthetic while performing the surgery, it might take about 30 minutes to recover from the sedation.

HOW TO PREPARE YOURSELF FOR THE CATARACT SURGERY?

Before undergoing a cataract surgery you may be asked to do the following:

  • A week before the surgery, the doctor may take certain painless tests using ultrasonic energy to determine the size and shape of your eye.
  • You may be advised to stop certain medications at the time of your surgery as that might increase the risk of bleeding during the procedure.
  • You will be advised to use antibiotic eye drops to reduce the risk of infection.
  • You will be instructed to fast for 12 hours before your surgery.

RISKS AND COMPLICATIONS OF THE SURGERY:

A person suffering from cataract alone may not have many complications, but those who have other eye conditions such as uveitis, high myopia (severe short-sightedness). Less than 10% of the people have greater chances of developing PCO, within two to three years after the cataract surgery.

 POSTERIOR CAPSULE OPACIFICATION (PCO): it is the condition in which the lens capsule thickens thereby causing cloudy vision. This isn’t the cataract developing again, it is the skin or membrane growing over the artificial lens

Other complications are less common but to list a few, they are:

During the operation:

  • inability to remove all of the cataracts
  • tearing of the lens capsule
  • bleeding inside the eye
  • a bit of the cataract dropping into the back of the eye
  • damage to other parts of the eye, such as the transparent outer layer of the eye (cornea)

After the operation:

  • swelling or inflammation of the eye
  • swelling and redness (inflammation) in the eye
  • swelling of the retina- where the fluid builds up between the layers of the retina at the back of the eye. This may sometimes affect the vision.
  • Swelling of the cornea- where the fluid builds up in the corner at the front of the eye; it usually clears itself.
  • retinal detachment – a rare complication where the retina (the layer of nerve cells inside the back of the eye) becomes separated from the inner wall of the eye
  • infection in the eye – such as endophthalmitis (a rare bacterial infection)

It is important to seek medical advice if you experience any increasing pain in the eye or loss of vision or redness after the surgery. It is possible to treat these complications with further surgery.

There is also a small risk of around 1 in 1000 may experience permanent damage to the eye resulting in loss of vision. However, the majority of the people lead a happy life with improved vision after the surgery.

STROKE- SYMPTOMS, TREATMENT, AND PREVENTION

Stroke or a brain attack is a condition that occurs when there is inadequate blood supply to the brain. Due to decreased blood flow and lack of oxygen in the brain, the brain cells begin to die off eventually. Stroke can be classified into 2 main categories:

Due to blockage of blood flow: Ischemic stroke, which is due to the blockage of a blood vessel in the neck or brain, is the most frequent cause of a stroke. About 80% of the stroke cases are a result of blockage of the blood These blockages arise due to 3 conditions:

  1. Thrombosis, which is the formation of a clot within the blood vessel in the neck or brain.
  2. The movement of the clot from other parts of the body such as heart to neck or brain
  3. Stenosis, a severe narrowing of an artery in or leading to the brain.

Due to bleeding: Hemorrhagic stroke, which is the bleeding into the brain or spaces surrounding the brain causes the second type of stroke.

Symptoms of a Stroke:

The following are certain signs and warning that you have high chances of getting a stroke.

  • Sudden falls or brief unconsciousness
  • Unexplained dizziness
  •  Sudden weakness or numbness on one side of the arm, leg or face
  •  Abrupt loss of vision, strength, coordination. Speech and ability to understand speech. These symptoms become worse over time.
  • Sudden dimness of vision, especially in one eye.
  • Sudden loss of balance accompanied by vomiting, nausea, and difficulty in swallowing.

The F.A.S.T test is usually taken to confirm if the patient is actually affected by stroke.

F means face– if one side of the face droops it’s a possible sign of stroke.

A means arm– if the person is not able to stretch out both arms then again it’s a possible sign of stroke.

S means speech– if the person is not able to understand simple sentences then again it’s a possible sign of stroke.

T means time- if any of the FAS signs are positive then its TIME. The T in FAST test also means the longer the brain is blocked from the blood supply, the greater is it’s damage caused. Usually, the patient affected by stroke should be treated within 3 hours.

What causes a stroke?

A condition called atherosclerosis may occur due to deposition of fats, cholesterol, calcium and other substances which narrow the blood vessel making it easy for the clots. This reduces the blood flow to the brain. As a result, the brain suffers from a lack of oxygen and nutrients. Some people may experience only a temporary disruption of blood flow to the brain. This is called transient ischemic attack (TIA).

TRANSIENT ISCHEMIC ATTACK (TIA):

A transient ischemic attack (TIA) also called as mini-stroke will have the similar symptoms of a stroke but for a brief duration only. TIA lasts for less than 5 minutes. TIA may also occur due to a clot or blockage that reduces the blood flow to part of the brain. Unfortunately, it’s not possible to detect if the patient is undergoing a TIA or a stroke.

Risk Factor of Stroke:

Stroke may certainly lead a person to have a severe heart attack. The following are some of the risk factors:

  • People above 5 years of age
  • Personal or family history of stroke
  • Men have higher chances of being affected by stroke than women.
  • African-Americans have a higher risk of stroke than people of other race.

Complications of Stroke:

The complications of stroke may be severe or mild depending on how long the brain lacked proper blood flow and also which part of the brain was affected. The complications may include:

  • Paralysis or loss of muscle movement
  • Difficulty in walking or swallowing
  • Loss of memory and thinking abilities
  • Difficulty in controlling their emotions
  • Pain, numbness or strange sensations in parts of the body.
  • Changes in behaviour and self-care ability.

Prevention of Stroke:

A stroke prevention is about maintaining a healthy lifestyle which helps to fight stroke every single day. In addition to preventing stroke, these actions will also help one to lead an energetic and peaceful life.
