  • High cholesterol can create a bile imbalance, leading to gallstones. According to the National Digestive Disease Information Clearinghouse, more than 80% of gallstones are cholesterol stones.
  • Very high cholesterol can lead to fatty deposits within your ligaments and skin, as well your liver and spleen. The enlargement of your liver or spleen due to fatty deposits places pressure on other organs within the abdominal cavity, leading to an unexplained stomach or abdominal pain.
  • When plaque builds up in the arteries that carry blood to the brain, the brain is deprived of oxygen. Brain cells quickly become damaged and start to die (stroke).
  • As coronary arteries narrow due to the cholesterol deposit, it’s harder for blood to make it to the heart. And if an area of plaque breaks open, it can result in a blood clot, which can block blood flow altogether. This puts one at a great risk of having a heart attack.
  • One of the main side effects of high cholesterol is a progressive narrowing of the arteries and hardening of the artery walls, a condition called atherosclerosis. This condition causes the arteries to narrow and harden, increasing the risk of an arterial blockage or blood vessel rupture.
  • Plaque can also build up and interfere with blood flow to the arms and legs (peripheral arterial disease). If the blood supply to the limbs is blocked, there may be numbness or pain. There’s an increased risk of infection in those limbs. Lack of blood can cause tissue death (gangrene) too.
  • Another potential long-term side effect of high blood cholesterol is an increased strain on your heart muscle due to the development of the cholesterol-linked vascular disease. When the arteries narrow the heart pumps harder to push blood through the blood vessels, often leading to an elevation in the heart rate. This puts a strain on your heart over time. Without the apt treatment, this heart strain may increase the risk of potentially fatal heart failure.

Enigma of cholesterol

Most of us feel that cholesterol are bad for health and try to avoid them. Few of us know that there exists good cholesterol too, which can do miracles for our body. Still fewer know the ways to increase this good cholesterol level in the blood and derive the benefits.

Cholesterol are a specific class of fat which are required in optimal quantity for maintenance of a healthy cell. If the intake exceeds, their concentration will rise in the blood stream causing the formation of plaques and finally resulting in arterial blockages. Those specific types of cholesterol which cause such blockages are the LDL or low density lipoproteins notoriously known as the bad cholesterol. Lower the concentration of LDL, lesser are the chances of arterial blockage and coronary artery disease.

There exists another type of cholesterol called HDL or high density lipoproteins which are popularly called good cholesterol. They carry the LDL away from blood stream to liver and promotes their destruction there. Thus, they act as a mediator to reduce blood LDL level. Higher the HDL, lesser will be the LDL, and lesser will be the chance of arterial blockage and coronary artery disease.

To keep a check on the LDL level, one should reduce the daily intake of fat in the form of butter, ghee and other processed dairy products. Vegetable oils used should be on rotational basis everyday. This means olive oil, soya bean oil, mustard oil, sunflower oil etc. should be used one after another on different week days. Regular exercises and yoga are also found to reduce LDL level in blood.

HDL are found to increase with regular exercises. A person more physically active has higher concentration of HDLs in the blood stream. Regular intake of almonds and walnuts increase the blood HDL level. Smoking reduces the HDL level.

Cholesterols are good as well as bad. You can like them; you can dislike them; but you cannot ignore them. So better ;manage them.

How Common is Male Infertility?

It is a very common problem. For 1 in every 5 couples, the infertility problem solely lies in the male. In most men, sperm antibodies will not affect the chance of a pregnancy but in some men sperm antibodies reduce fertility.

Male Reproductive System:

There are 2 main glands that help in the production of male hormones and sperms. They are the pituitary and the hypothalamus glands. Luteinizing hormone and follicle-stimulating hormone are the 2 important messenger hormones made by the pituitary gland that acts on the testes.

The tests are a pair of egg-shaped glands that sit in the scrotum next to the base of the penis on the outside of the body. The testes make sperm and the male sex hormone testosterone. It takes about 70 days for sperm to become mature and able to fertilize an egg. When released from the testes, the sperm spends two to 10 days passing through the epididymis where they gain the vital ability to swim strongly or become motile and to attach to and penetrate the egg.

The waves of muscle contractions transport the sperm, with a small amount of fluid, from the testes to the vas deferens. The seminal vesicles and prostate contribute extra fluid to protect the sperm. What is Erectile Dysfunction?

A man is said to have erectile dysfunction if he has permanent difficulty in getting or maintaining a firm enough erection to be able to achieve sexual activity.   • Treatments for prostate disease
  • Surgical complications (surgeries include radical prostatectomy, cystectomy, transurethral resection of the prostate and rectal cancer surgery)
  • Injuries in the pelvic area or spinal cord
  • Radiation therapy to the pelvic region.

Numerous medicines can also cause erectile dysfunction. Anyone taking prescription medications should consult their doctor or healthcare practitioner prior to stopping or changing their medications.

  • Drugs to control high blood pressure
  • Heart medications such as digoxin
  • Some diuretics (known as ‘water pills’ that increase urine output)
  • Drugs that act on the central nervous system (in the brain or spinal cord), including some sleeping pills and amphetamines
  • Anxiety treatments (anxiolytics)
  • Antidepressants including monoamine oxidase inhibitors (MAOIs), selective-serotonin reuptake inhibitors (SSRIs), and tricyclic antidepressants
  • Opioid painkillers
  • Some cancer drugs, including chemotherapeutic agents
  • Prostate treatments, including treatments for cancer, and for benign prostatic hypertrophy (such as 5α-reductase inhibitors). Anti-androgen drugs used for prostate disease and other conditions can also cause erectile dysfunction
  • Anticholinergics – drugs with a wide range of uses that block a neurotransmitter (nerve signal molecule) that affects muscle tone
  • Hormone drugs, including estrogens and drugs that act on luteinizing hormone-releasing hormone
  • The peptic ulcer medication cimetidine

CATARACT SURGERY: TYPES, PROCESS & RISKS

WHAT IS CATARACT??

A cataract refers to the clouding of the eye’s natural lens which lies behind the iris and the pupil. It is the principal cause of blindness in the world and is most common in people of age 40 years and above.

There are 3 types of cataract:

  • SUBCAPSULAR CATARACT: This type of cataract occurs at the back of the eye. It is most common in people with diabetes or those taking high doses of steroid medication.
  • NUCLEAR CATARACT: This type of cataract occurs in the deep central zone of the lens. It occurs due to ageing.
  • CORTICAL CATARACT: This type of cataract begins in the periphery of the lens and slowly spreads towards the centre of the lens. It affects the cortex of the lens, which is the part of the lens that surrounds the central zone.

CATARACT- IS IT BECAUSE OF YOUR FAMILY, LIFESTYLE OR JUST BAD KARMA??

Our eye lens works very much like the camera, focusing light onto the retina for clear vision. It also adjusts the eye’s focus enabling us to view the things both close up and far away. The eye lens is made of water and certain other proteins that let light to pass through it. With ageing, some of the protein may clump together and cloud up over a small area. With time, the clouding up becomes larger and larger making it harder to see. This condition is called cataract.

Nobody is pretty sure why the eye lens changes with age resulting in the cataract. Researchers have eventually found out other risk factors that may lead to cataract. Some of which are:

There’s no concrete proof on what could reduce the risk of cataract. There is no harm in practising a healthy lifestyle that consists of a diet rich in nutrients, vitamins, minerals, antioxidants and omega 3 fatty acids. Regular exercise and sound sleep also add on to your quality of life.

Though it seems to be terrific, it is something we needn’t worry a lot about! The advancements in the field of medicine have made it possible to cure the patients of cataract with help of surgeries.

CATARACT SURGERY:

Cataract surgery is the process of removing the natural lens of the eye (crystalline lens) which is affected by cataract. During the cataract surgery, the patient’s cloudy lens is removed and is replaced with synthetic lens ensuring lens’s transparency. The synthetic lens replaced is otherwise called as INTRAOCULAR LENS (IOL). The cataract surgery is performed by an ophthalmologist.

There is two commonly preferred type of surgery for cataract. They are:

  • PHACOEMULSIFICATION
  • MANUAL SMALL INCISION CATARACT SURGERY (MSICS):cataract, healthy eye

Phacoemulsification is the most commonly preferred method for cataract surgery. It involves the use of ultrasonic energy to emulsify the cataract lens. It comprises of 6 steps:

  • ANAESTHETIC: Using topical anaesthetic eye drops or subtenon injection, the eye is numbed.
  • CORNEAL INCISION: two cuts are made at the margins of the clear cornea that allows insertion of instruments into the eye.
  • CAPSULORHEXIS: With help of a needle or a pair of forceps, a circular hole is made in the capsule in which the lens is placed.
  • PHACOEMULSIFICATION: Ultrasonic energy is used to emulsify the lens into a liquid. The resulting emulsion is then “sucked away”.
  • IRRIGATION & ASPIRATION: The cortex, which is the outer layer of the cataract is sucked away. The fluid removed is continuously replaced with saline solution to prevent a collapse of the structure.
  • LENS INSERTION: A plastic, foldable lens is placed in the capsule which contains the natural lens. Along with this, an antibiotic is also injected into the eye to reduce the risk of infection.

Finally, salt water is injected into the corneal wounds, which cause the area to swell and seal the incision.

MANUAL SMALL INCISION CATARACT SURGERY (MSICS): This technique is evolved from ECCE. The word “small” in the title refers to the wound being smaller than an ECCE, although it is larger than a PHACO wound.

HOW LONG DOES IT TAKE TO PERFORM A CATARACT?

If it is an uncomplicated surgery, then it will not take more than 10 minutes. Since the patient will be given anaesthetic while performing the surgery, it might take about 30 minutes to recover from the sedation.

HOW TO PREPARE YOURSELF FOR THE CATARACT SURGERY?

Before undergoing a cataract surgery you may be asked to do the following:

  • A week before the surgery, the doctor may take certain painless tests using ultrasonic energy to determine the size and shape of your eye.
  • You may be advised to stop certain medications at the time of your surgery as that might increase the risk of bleeding during the procedure.
  • You will be advised to use antibiotic eye drops to reduce the risk of infection.
  • You will be instructed to fast for 12 hours before your surgery.

RISKS AND COMPLICATIONS OF THE SURGERY:

A person suffering from cataract alone may not have many complications, but those who have other eye conditions such as uveitis, high myopia (severe short-sightedness). Less than 10% of the people have greater chances of developing PCO, within two to three years after the cataract surgery.

 POSTERIOR CAPSULE OPACIFICATION (PCO): it is the condition in which the lens capsule thickens thereby causing cloudy vision. This isn’t the cataract developing again, it is the skin or membrane growing over the artificial lens

Other complications are less common but to list a few, they are:

During the operation:

  • inability to remove all of the cataracts
  • tearing of the lens capsule
  • bleeding inside the eye
  • a bit of the cataract dropping into the back of the eye
  • damage to other parts of the eye, such as the transparent outer layer of the eye (cornea)

After the operation:

  • swelling or inflammation of the eye
  • swelling and redness (inflammation) in the eye
  • swelling of the retina- where the fluid builds up between the layers of the retina at the back of the eye. This may sometimes affect the vision.
  • Swelling of the cornea- where the fluid builds up in the corner at the front of the eye; it usually clears itself.
  • retinal detachment – a rare complication where the retina (the layer of nerve cells inside the back of the eye) becomes separated from the inner wall of the eye
  • infection in the eye – such as endophthalmitis (a rare bacterial infection)

It is important to seek medical advice if you experience any increasing pain in the eye or loss of vision or redness after the surgery. It is possible to treat these complications with further surgery.

There is also a small risk of around 1 in 1000 may experience permanent damage to the eye resulting in loss of vision. However, the majority of the people lead a happy life with improved vision after the surgery.

  • LH further binds itself to the target cells which may again lead to an increase in the extraction of steroidogenic acute regulatory protein (StAR) which promotes cholesterol transfers and initiates steroidogenesis.
  • Androgens, DHEA and androstenedione are released in the adrenal cortex.
  • Testosterone is released in the Leydig cells, lying adjacent to the male testes.
  • The production of pregnenolone is initiated in the Leydig cells of the luteinizing hormone.
  • The synthesis of oestrogen in females helps in controlling the growth and development of sexual characteristics in females.
  • The synthesis of glandular oestrogen takes place in the ovarian cells and the corpus luteum.
  • The luteinizing hormone (LH) stimulates the granulosa cells to release pregnenolone.
  • Pregnenolone then diffuses from these granulosa cells and mediate the process of pregnenolone hormone conversion to androstenedione through DHEA.
  • The process of extra-glandular synthesis facilitates the aromatization of androgens to oestrone.
  • The fat cells convert the androgen to oestrone and thereby increases the serum oestrogens.
  • The bones convert testosterone to form local oestrogen and thereby supports the development of epiphyses.
  • Even though oestrogens are usually responsible for balancing the sexual characteristics in females, however, they are also synthesized at some levels in males.
  • Progestin synthesis also exists. The inventories of hormone synthesis alter menstrual cycles during its production.

Sex Hormones: Regulation

  • The release of GnRH regulates the secretion of few hormones like the luteinizing hormone (LH) and the FSH during the movement and circulation.
  • The luteinizing hormone (LH) regulates the release of testosterone from the related cells and stimulates its production.
  • The testosterone released has a negative effect on the luteinizing hormone (LH) and the hypothalamic GnRH
  • The FSH regulates the stimulation and production of inhibin from the related cells.
  • The released inhibin then again feeds an anterior pituitary in return to produce FSH.
  • The regulation of hormones in females may be considered a bit complicated as compared to male hormones regulation. The reason is mainly that of the effects of various hormones that vary during the stages of menstrual cycle.
  • In males, the hypothalamic gonadotropin-releasing hormone (GnRH) leads to the production of LH and FSH next to the anterior pituitary.
  • Feedbacks from the theca cells are regulated by the production of the progestins.
  • Feedbacks from the granulosa cells are regulated by the production of the inhibin.
  • Feedbacks from the corpus luteum are regulated by the production of the progesterone and oestradiol during the menstrual cycle phase.
  • Any higher amount of the production or release of the progesterone and oestrogen takes place due to the corpus luteum. As a result, the release of LH and FSH are inhibited.
  • Further, the levels of few hormones like specifically the LH and the FSH tend to increase to some higher levels and thus a decrease in the levels of oestrogen and the progesterone takes place whereby causing the occurrence of the certain new follicle.
  • The follicle that is in the growing stage starts to release oestrogen in heavy amounts which thereby also affects the release of LH and FSH
  • The sudden increase of LH and the FSH takes place which then prepares for the process of ovulation. This mainly happens due to a certain increase in the production of oestradiol or the new follicle that gets produced to higher levels.
